Would you like to have your say about services at Manor Park Surgery?
We are setting up Manor Park Surgery Patient Participation Group.
We are aiming to meet approximately once every 4-6 months. Together we will shape a patient survey and publish its results.

Q Why are you asking people for their contact details?
A We would like to be able to form a patient participation group to devise a patient survey and discuss its results.

Q Will my doctor know I am involved?
A This information is purely to contact patients to invite them to get involved. Your doctor may attend the planned patient participation meeting and they may see general feedback from patients.

Q Will you ask me personal information?
A We will only discuss general information about the practice.

Q who else will be able to access my contact details?
A Your contact details will be kept safe and secure and will only be used for this purpose and will not be shared with anyone else.

Q How often will the group meet?
A Not very often, approximately once every 4 to 6 months.

Q What is a patient participation group?
A This is a group of volunteer patients who are involved in making sure the surgery provides the services its patients need.

Q Do I have to leave my contact details?
A No, but please let us know if you change your mind.

Q What if I no longer wish to be involved?
A We ask that you let us know if you do not wish to receive further contact.
