As a patient under the GMS contract you can expect to receive an appointment within
48 hours with a doctor (not necessarily the doctor of your choice). We always
endeavour to meet this target; however, there will inevitably be times when it
is not possible.
All emergencies will be seen according to need. There
is a great demand for appointments here at the Manor Park Surgery and it would
help greatly if you let us know if you are not able to keep your appointment.
This appointment could be used by someone else. Help us to help someone
else. Patients who do not attend for their appointment on three separate
occasions will be asked to register with another practice.
To make an appointment,
please call 0844 477 3145. The telephone system we have will guide you to the
appropriate department ie appointments, home visits, enquiries etc.
In
life-threatening emergencies such as severe bleeding, collapse, unconsciousness
and severe chest pains please telephone 999.
If you need to send a fax, please contact us on 0844 477 4009.
If there are any changes in your circumstances, ie, new address, telephone number, please contact the surgery so we can keep your records updated.
Out-Of-Hours
When the surgery is closed, from 18.00 to 08.00 the next day, Monday to Friday
and between 18.00 on Friday and 08.00 Monday, the out-of hours service is provided
by Local Care Direct, who have a team of doctors and nurses who are able to respond
to your needs. If you have cause to contact them for advice all you need to do
is contact the surgery on 0844 477 3145 and your call will be transferred directly
to them.
The surgery is also closed on one afternoon every month for staff training. All calls will be automatically transferred to Local Care Direct.
Please use this service wherever possible. DO NOT attend the A&E department for minor ailments - this is for
genuine accidents and emergencies only.
There are other options available for less serious cases. These include:
- NHS Walk-in Centres: The nearest facility is based at The Light, The Headrow, Leeds. It is open Monday to Friday 0700-1900. Telephone 0870 818 0033
- Minor Injury Units: There are two in this area. They can be found at:
| 1. St George's MIU, St George's Road, Middleton. It is open Monday to Friday 0800-2300. Telephone (0113) 392 9800. |
| 2. Wharfdale MIU, Wharfdale General Hospital, Newall Carr Road, Otley. It is open seven days a week 0800-2300. Telephone (01943) 456222. |
- NHS Direct: This is a 24-hour nurse led advice and information service which you can telephone on 0845 4647 or visit online at www.nhsdirect.nhs.uk
- Pharmacies: Your local pharmacist is highly qualified and can give advice if you are unsure whether you can treat your symptoms with 'over the counter' medication or do indeed need to see a GP.
Disabled Access
Our surgery has been designed with the disabled in mind. The surgery entrance
and consulting rooms are suitable for wheelchair access and there is a disabled
toilet. The car park meets with the Health Authority's requirements for a practice
of our size.
